back in 2000 I contacted Elma, a Russian company, and they had 5" mask blank at
$15,
the quality looked damn OK (just the method of payment was very formal, with
contract signature, but it seems that it changed), but finally I could recycle
some old mask and did not need them anymore :-)

(note that they want to sell big numbers - and then it costs even less than $15,
$12 if I remember - but I was able to order 20 blanks without problems)
