Try American Piezo Ceramics.  I think they are in the Thomas Register.
They may only go to 10 mil (250 mu-m).  If you need to polish to get your
desired thickness, I would say look somewhere else for raw materials (there
must be other makers out there).  Polishing bulk wafers will frequently
depole them.
