small, 2-axis accelerometer needed

Does anyone know of a source for small, 2-axis accelerometers? In this
particular case, small means the entire accelerometer/electronics package
(or even an unpackaged die containing the complete
accelerometer/electronics system) can be no larger than 2mm x 2mm and draw
no more than about 0.5 mA.

We need a system that can measure up to a few g's with a resolution in the
mg range and measure from DC to a few kHz (ms acquisition time) so we can
use it for inclination as well as acceleration.

As an example of something that was satisfactory in almost every way but
size (it's too big), we've looked at the Analog Devices ADXL202E.

Thanks in advance.
