Corning is the vendor for 7740. But they seemt to sell it exclusively in
wafer format. Though if they're willing to do custom work.

Corning also has a product class they call Pyrex Sheet, which curiously has
physical properties that are IDENTICAL to 7740:

Now the trick is getting the Pyrex Sheet in the shape and thickness you
want. Corning does make microscope slides, but it's not clear what glass
they use. If you look at their Life Sciences area, they say that 7740 is
great for laboratory glassware because of it's low thermal expansion.

If you're lucky, regular Corning microscope slides are made from 7740.
You'll probably have to talk to a technical representative on the phone to
find out for sure. If not, then you're reduced to finding someone that can
saw the glass to the proper size.
