I am with the optics group of Shipley Co., formerly Haleos, Inc.  We have
experience in making such things out of Silicon (Silicon Optical Benches), but
they are all customer specified designs and there is no commercial off the shelf
version available from us.  If there is a design you are interested in having
built, we can probably do it, but it will not be cost effective for just a
handful of parts.  If you are looking a higher volume requirement, it might make
sense.
